Your cooperation by using this form for disputes will help us to streamline the process, while also helping us to have the information we need to help you. Please do not send disputes directly to Donna Maltz, Vernon Jones or Manny Reyes. They will just send you to this dispute form. Instead, please use the form below. We will do out best to research your issue. If we cannot settle this for you, then it will be time to contact the promoter. If you are not happy with the result with the promoter, that is the time to ask for the sanctioning body to make a decision. 

Before filling out this form, please use the "Check/Verify Individual Member's Points" option on the Points Page. If you find a problem, please use this form to report the issue. Be sure to have all required information available before filling out the form. If your issue cannot be resolved at this level, you will be sent further instructions regarding your options.

NOTE: All fields are required. If you do not have the required information, please return to the Points Page where you can use the drop-down menu to "Check/Verify" to find the needed "MemberID" and "Division Number/s". If the division number that you need is not there, please use the drop-down menu for "Check Ranking In Any Subdivision". Between these two drop-down menus, you should be able to find what you need.
